One Source – Empowering Caregivers is a nonprofit organization that provides respite care for caregivers, at no cost.  We are conducting a survey to gain perspective about the stresses and concerns that caregivers face.  Your participation may lead to OSEC’s outreach program CARES (Community Awareness Resource Educational Series) enhancements.

This survey will take less than 10 minutes to complete.  Your responses are completely private and confidential.  Data will only be reported in aggregate (anonymous) form, and any personally identifiable data collected will be stored separate from survey responses and destroyed upon the completion of this project.

Your participation is voluntary, and you may skip any questions that you do not wish to answer.  For our purposes, the caregiver is an unpaid individual, who is assisting another in daily living activities at home.  If you are caring for a family member or friend at home, please take the time to fill out the survey.  You can make a difference in helping us find out the value of this service in our community.
